Where Do I Find Forms?
There are many different forms used by the Auxiliary to record missions, mission hours, competencies completed, member data, and other information. The majority of forms are located on the National Forms Site, while a few others are found on the District 13 site and the Division 6 site.
National Forms
This is where you will find the forms we all know and love like the 7028 – Change of Member Information; 7029 - Member Activity Log (“99”); 7030 – Mission Activity Report; 7038 – Vessel Examination Activity Report. There are a number of other forms that are used less frequently. If someone indicates that you should fill out a form starting with the number 7 or the code “CGAuxA”, this is the place to find the form. The SF-86 security form is also found here.
The National Forms Site also contains a select group of forms called “EMAIL FORMS”. The main forms used by members are here: the 7029, 7030, 7038. You can fill these forms out on-line, then enter your FSO-IS’s e-mail address at the bottom of the form and a summary copy of the form is e-mailed to the recipient. More information about using these forms is found later in this document.
District Forms
The District 13 website contains some forms and information
specific to D13. It also contains links
to the same forms listed on the National Forms Site. The National Forms are the same copies found from the National
site, but there are no links to the “EMAIL FORMS”. You can use the D13 forms page for most of your needs if you do
not want to use the special email forms.

This has routing information for many forms. But it is missing the D13 specific forms like the D13 ATON form and the Annual Currency Maintenance form.
